Welcome to IAFMHS Electronic Registration Information

 

Please note: All registrations must be completed online via IAFMHS Website. The system does not allow for purchase orders/invoices. You will need to email your purchase order to our office, tmoropito@iafmhs.org. There is a $25 (inlcuding GST) administration fee for all invoices. 

 

Billing Info: If you are a new customer please click go to checkout or if you are an existing member/customer enter your email or your customer/membership id number and password.

 

If you have ever registered for an IAFMHS conference or were a member at one time then you have an existing Order ID/Membership Number. Please use your email address to register/order if you do not remember your ID Number, or email that you used to previously register, please email us at: tmoropito@iafmhs.org to receive your ID/Membership Number and password. Creating a second order will cause delays with your registration and will create additional records in our system. If in doubt please email us prior to registering.

 

If you are new customer you will need to enter your contact information and payment method. If you an existing member you will need to enter payment method. Please click verify order.

 

If all your information is correct please click complete order and a receipt will appear on your screen.

 

If you are not a paid in full current member you will be charged the non-member rate - once you hit submit you will automatically be charged so please ensure you have made the correct selections.

 

Please note: You are only registered once payment is received.

 

Registration will be processed upon receipt of the appropriate fee, which can be processed by VISA, MasterCard, American Express, Canadian Cheque.  Please make cheques payable to IAFMHS and mail to:

All cancellation requests received in writing (email/fax) on or before April 25, 2010 receive a refund less a $50 + GST CAD administration fee. Refunds will not be considered after this date. Refunds do not apply to conference banquet.
